Report a non-standard school transition

Before you start...

This form will allow you to notify us of the non-standard transition of a child to another school. To complete the form you will need to collect the following information:

  • Details of parents / guardians including date of birth if known
  • School details for the previous and destination schools, including the enrolment date for the destination school, the last attendance date of the previous school and the date the child was added and removed from each school roll
  • Contact names and telephone numbers of any services or agencies involved with the child
