This form will allow you to refer a child who has been missing education. To complete the form you will need to collect the following information for entry or upload:
- Details of parents / guardians including date of birth if known
- A copy of the current attendance register
- Contact names and telephone numbers of any agencies involved with the child
- Details of siblings, including name, date of birth and school, if known
- Details of recent action taken by the school or referrer, including copies of any correspondence
